Are you interested in a “housing first” approach to homelessness? Then the Housing Stabilization Case Manager position is for you!
The Housing Stabilization Case Manager provides direct support to individuals who were formerly homeless and have transitioned into permanent housing. The Housing Stabilization Case Manager operates from a “housing first” point of view, working closely with recently housed residents to ensure they have the support to develop, apply skills, and access to resources needed to live independently in stable permanent housing.
The Housing Stabilization Case Manager uses resources within community, St. Francis House and housing department to provide goal orientated case management services to the residents on their caseload. The goal of the Housing Stabilization Case Manager is helping individuals achieve success in the areas of permanent housing, income maximization and behavioral health stability.
